Form 1, line 10?

I am filling out my form 1 with a RLT (Trust). This is my first time doing a SBR with a Trust. On line 10 of form 1, after the Signature of applicant it asks for “Name and title of Authorized Offical of Firm or Corp”. Do I put my trust title there or do I leave it blank ?

Ex. The RobertJ RLT, RobertJ"Trustee" (or what?)

I put… “My name, Grantor”

I had the same question that you do! Of course, I’m still waiting for my approvals to return.

You can use either of the 2. But if your Trust say’s Trustee, I’d go with that just in case…:wink:

Well… I got the first of two Form 1s back approved with my name, "Grantor" on them.

So I guess that was acceptable.
