CREATINE: Everything You Need To Know

Well almost everything. These are the most common Qs I get, and I have done an extensive FAQ on creatine for readers to cut through the often confusing BS on this supplement:

What does creatine do?

In a nutshell, creatine works to help generate cellular energy. When ATP loses a phosphate molecule and becomes adenosine diphosphate (ADP), it must be converted back to ATP to produce energy. Creatine is stored in the human body as creatine phosphate (CP) also called phosphocreatine.

When ATP is depleted, it can be recharged by CP. That is, CP donates a phosphate molecule to the ADP, making it ATP again. An increased pool of CP means faster and greater recharging of ATP, which means more work can be performed. This is why creatine has been so successful for athletes. For short-duration explosive sports, such as sprinting, weight lifting and other anaerobic endeavors, ATP is the energy system used.

To date, research has shown that ingesting creatine can increase the total body pool of CP which leads to greater generation of energy for anaerobic forms of exercise, such as weight training and sprinting. Other effects of creatine may be increases in protein synthesis and increased cell hydration.

Creatine has had spotty results in affecting performance in endurance sports such as swimming, rowing and long distance running, with some studies showing no positive effects on performance in endurance athletes. Whether or not the failure of creatine to improve performance in endurance athletes was due to the nature of the sport or the design of the studies is still being debated.

Is creatine bad for me?

Creatine the most studied nutritional supplement that exists. Hundreds of studies to date have shown that creatine monohydrate is an amazingly non-toxic and safe supplement with numerous benefits. Further studies directly examining possible side effects, both prospective and long-term retrospective (up to five years), have failed to find any serious side effects of creatine supplementation (1-5) on various markers studied, such as renal function, hepatic function, and others.

Contraindications of creatine monohydrate:

Although creatine monohydrate is clearly safe for healthy people with a very low side-effects profile using up to 10 grams per day, are there specific groups who should not use it?

Again, the data suggest very few actual contraindications. The population who should avoid creatine supplements are those with a history of renal disease and/or those taking nephrotoxic (poisonous to the kidneys) medications. There’s been a handful of case reports that show very high doses of creatine (and the reports were not always clear as to what form of creatine was used) were associated with kidney dysfunction.(70) Typical for such a simple case report, it’s unclear what other medications were involved or pre-existing medical condition existed.

However tenuous the connection between high-dose creatine monohydrate and pre-existing kidney dysfunction, it’s prudent to advise people with a history of renal disease and/or those taking nephrotoxic medications to avoid creatine supplementation until more data exists examining that connection. As creatine monohydrate supplementation may cause a transient increase in creatinine levels in some individuals, it may act as a false indicator of renal dysfunction.

How should I take creatine, capsule/powder/with food/morning/afternoon/night?

Most cost effective way to take creatine is as a powder mixed into a warm liquid. Mixing in a warm liquid may not impact absorption and utilization, pre solubilizing creatine generally eliminates any gastrointestinal some may experience. Early studies mixed creatine hot tea and coffee. Timing does not appear to matter, although one small study suggested post workout was superior. Most take creatine post workout, but as creatine is a chronic effect (due to increased tissue levels of PC) vs. an acute effect such a caffeine does.


Should I/How should I take creatine if I only lift weights occasionally?

Because creatine works by elevating tissue levels of CP, vs an acute effect on any one workout (like taking a stimulant before a workout) creatine should be taken daily to maintain elevated tissue levels. Once higher tissue levels are reached, via a simple intake of 3-5g per day for at least 30 days, or a loading phase done, followed by maintenance dose, it’s possible creatine does not need to be taken daily. However, what dose at what schedule to maintain tissue levels is unclear at this time. I recommend simply taking 3-5 daily. Does creatine help with muscle mass or overall health?

Yes and yes. Creatine does both assist in building muscle and or preserving muscle, as well as other tissues. Most know creatine as a “muscle building supplement” but are unaware of it’s potential health benefits and medical uses. If I didn’t exercise at all, I’d still use creatine personally. Creatine is proving to be one of the most promising, well researched, and safe supplements ever discovered for an exceptionally wide range of uses.

What are the downsides to creatine?

There’s a risk/benefit to anything and everything we do, but I’m aware of no serious downsides that would outwigh the benefits in the vast majority of users at this time. Most reported downsides, such a dehydration or cramping and such, are mythology that has been studied and debunked in the studies, which actually found a slightly reduced rate of cramping and improved thermoregulation with creatine as an example.


What’s the best form of creatine?

Creatine monohydrate (CM) form is far and away the most extensively researched. To date, every “alternative” to CM claiming to be superior, once actually tested in a research setting has failed miseralbly to show any superiority to CM. Section One

Creatine is formed in the human body from the amino acids methionine, glycine and arginine. The average person’s body contains approximately 120 grams of creatine stored as creatine phosphate.
Certain foods such as beef, herring and salmon, are fairly high in creatine. However, a person would have to eat pounds of these foods daily to equal what can be obtained in one teaspoon of powdered creatine.
Creatine is directly related to adenosine triphosphate (ATP). ATP is formed in the powerhouses of the cell, the mitochondria. ATP is often referred to as the “universal energy molecule” used by every cell in our bodies.
An increase in oxidative stress coupled with a cell’s inability to produce essential energy molecules such as ATP, is a hallmark of the aging cell and is found in many disease states.

Key factors in maintaining health are the ability to:

Prevent mitochondrial damage to DNA caused by reactive oxygen species (ROS)
Prevent the decline in ATP synthesis, which reduces whole body ATP levels.

It would appear that maintaining antioxidant status (in particular intra-cellular glutathione) and ATP levels are essential in fighting the aging process.

It is interesting to note that many of the most promising anti-aging nutrients such as CoQ10, NAD, acetyl-l-carnitine and lipoic acid are all taken to maintain the ability of the mitochondria to produce high energy compounds such as ATP and reduce oxidative stress.

The ability of a cell to do work is directly related to its ATP status and the health of the mitochondria. Heart tissue, neurons in the brain and other highly active tissues are very sensitive to this system.

Great write up. This has dispelled some of the myths that surround CM. I’ve been using this stuff for years and I agree that it’s not just snake oil.

I have taken all types of supplements since I was 18. I have taken Creatine a few times sporadically. But for some reason it’s been the one that I have neglected.

I really think it was because I saw really good progress through heavy lifting and high protein diets. AND, every BRO in the gym asks you if you’re on creatine when you’re a muscular dude. I guess that has given it a stigma to me.

I guess I subconsciously I like saying no in regards to taking creatine(since everyone thinks you take creatine and automatically get huge.)

I do think I will begin supplementing creatine after this piece, as it’s been about 5 years since I last tried it.

Absolutely, I think more than anything this piece just made me think about why I have avoided it. But reading this brought some self-realization on my thoughts on it. And they’ve been short sighted. I have known it to be a healthy Low/No side effect supplement for a long time but just never used it.

There is seemingly(except for the individual problems some people have) no reason to avoid it. I’m glad you took the time to write this out for everyone.

great report Will. I consistently have used creatine monohydrate for years mainly for the health benefits and also improved performance. My liver and kidney tests have always came back fine - the only negative side effect that I’m more prone to while using CM is muscle spasm / cramping. Important thing to remember with CM is its just a supplement and maybe a plus 5% over not using it - keep expectations realistic and don’t expect an amazing Test C and deca type incredible anabolic response.

Will,

Since last Thursday, I have been in a loading phase with Creatine monohydrate. I have continued with my normal lifting routine, but notice my muscular soreness has elevated noticeably from my pre-creatine levels. Is there any reason I would be more sore after using creatine than before?

Is the loading phase inhibiting my kidneys from flushing the normal amount of lactic acid or something?

I have the same feeling of soreness as if I haven’t worked out in a couple months and hit it hard the first day back. but I have been on the normal routine for months.

Creatine is not known for increasing DOMS per se, so not sure what or if there’s a connection there. You don’t have to load if you don’t want to of course.

It’s not a terrible soreness, just the only real side effect that I’ve noticed that may not even be attributed to the creatine consumption. I just thought I’d ask if there were any explanations as this is the only thing that I have changed.

Well, one could hypothesize that the cell swelling effects of creatine loading might increase DOMs or something along those lines, but it’s not been reported in the lit, so pretty much a WAG on my end. Perhaps you’re just training a tad harder from the addition of the creatine.

Been wanting to try creatine…but the reports of accelerated hair loss make me hesitant.

I’ve been taking creatine since 4/30. So about 3 weeks.

I have noticed an increase in muscle size. Mainly in the bi’s/tri’s due to the shirt sleeves becoming tighter. I dont have any measurements but notice all around size increase.

I don’t “max out” on anything anymore due to fear of injury. However, on barbell flat bench I was at 255# for a set of 8 as my max prior to creatine. I’ve only had 2 chest workouts since starting and yesterday with no spotter I was able to get 9 reps at 255#. I only bring up no spotter because I believe I could have gotten 10 but when I am in a gym alone(no one actually at the gym) and have no means to call for help I didn’t want to risk not being able to get the 10th up. Hopefully next chest day someone will be in the gym and I’ll be trying for 10.

When I was in the Corps and actually maxing out on stuff because I was 23, my max was 325# on bench and whatever weight I did for reps and plugged in to this calculator correlated well.

Sounds like creatine is GTG for you! The the 1 Rep Calculator above gives their sources and likely a decent approximation of the 1RM with a margin or error. There’s too many variables to getting a really accurate 1RM calculated from number of reps with X % of 1RM it’s a rough guide at best.

I only ask because the caveman in me says “try 340#, the 1 rep max calculator says your there.” But the person who doesnt want to get hurt in me says “it’s not worth it.” :wink:

Well, if you expect say a 10-20lbs margin of error, might be 320 or 360. End of the day, no way to know 'till you try it, assuming you want to be able to answer the ever popular “how much can you bench?” Q which is the eye roller Q of all long time lifters.

Even these days, knowing about what I can do, I always answer that I don’t max out any more. Not to be cool, but because I really don’t. I also don’t wear Beats by Dre in the gym with color coordinated attire.

I tend to avoid those that do personally…I don’t go below 6 reps these days, and even that’s rare. Maybe one week a month I’ll do a few sets at around 80% 1RM which is approx 6 reps but still stop a rep short of failure. I’m much more concerned with longevity than max strength these days personally. My days of 500lb squat/deads and 400lb bench, loooooooooooooooooong gone.
