I just installed a 12in Daniel Defense Rail on a 14.5 in colt upper. Went on great, the pins holding the FSP were a pain to drive out. Looks good and is lighter and balances better than the KAC rail that came with it. I run a surefire x-300 n the 12’ and it works great for me. Midway usa had all the parts for installing the rail in stock. On a side note, got some guys here with me (sandbox) who do not like or run P-mags they say that they are extremly sensitive to falling out of the M-4’s during CQB type ops. I run them and the only issue I have found is that the mag release requires alot less pressing in with p-mags than with GI mags.

Did you use the RIS II with FSP cutout?
As far as the PMag issue, I’ve never heard of that issue. I’ve been running Pmags since Aug 07, and have used or seen others use several hundred copies. The only time I’ve seen mags fall out, is when they aren’t seated properly from the start. Why would they only fall out during CQB?
I don’t see why pmags would have issues falling out.
On that note, I do have limited experience with them as most all the ones I have (early generation I think) fit tight in my lowers, and I prefer the way GI mags slide in and out better.

Is it for sure the mags causing the issue, or something with the mag release?
I’m willing to bet the PMag problems are more training issues than equipment issues, otherwise the PMags or rifles would be failing all the time vs when just doing “cqb type ops”.
On that note, I’d love to see the mods to your M-4.
