You’re wanting the info for 4a? If so, just what’s engraved on the lower. Some of mine have name, then city, state…some even have name and just United States.

Do I need to put my EIN on an SBR form?
Sections 5 & 6? Not unless you’re an FFL/SOT. And even then, I wouldn’t think so since you’re filing as an individual and not the company.
Will I have to engrave my rifle before or after I get the approval from the ATF?
It needs to be marked when it’s configured as an SBR. So, nothing to do with before/after approval, but more with before/after the making occurs.
I’ve always engraved mine while waiting for approval. Some wait until afterwards. Some engrave before they even file.
You may wish to wait until the approval comes before engraving the rifle. Sometimes the ATF disapproves and suggests changes to the engraving verbiage.
