In heavenly love abiding, no change my heart shall fear;
and safe is such confiding, for nothing changes here:
the storm may roar without me, my heart may low be laid;
but God is round about me, and can I be dismayed?

Well that narrows it out to be a no-sh$t failure (which you pointed out in your earlier disruption.) vs just putting it on in a matter they would cause it to fail.
I'd like to know what their explanation on the MAAD system failure. I didn't see it in the thread but did you have a baffle strike?

If you tighten the mount down in your hand, do all three ears inside the MAAD still come out?

Not on mine anymore. They did up until the event.
As luck would have I was just at the range working with my wife and her handgun, when a fairly regular customer with a 7.5" barreled SBR and Saker762 setup began shooting in the lane next to us. The first round launched the suppressor down range and damaged his muzzle brake with the same distinct marks that are on my flash hider. We called a cease fire, retrieves the suppressor and the MAAD mount had sustained the impact. Reinstalling the suppressor yielded that it still locked up correctly, but would no longer stay on the muzzle device because the shoulder the suppressor locks on to had all been rounded off. I'm now of the belief that this is not an isolated incident.
